Physicals
A valid medical physical is required for any student wishing to participate on an athletic team. Physicals are good for one school year and you only need an updated medical physical if your current one is expiring before the start of the sport season.
If you are unsure of when your physical will be expiring, please contact the nurse’s office using the contact information below.
- All students must have a current physical on file with the nurse's office for sports participation.
- Physicals are valid for one full year (365 days).
- If a physical expires during the sport season/after their first practice, the student’s medical eligibility extends until that season is completed.
- Any student who requires medications for Asthma, Allergies, Diabetes, or Seizures MUST submit an ESM Medication Form. The self-carry section of the Medication Form MUST be completed by a physician and signed by a parent in pen. Students will not be cleared for sports participation without this completed form.
All forms must be submitted to the nurse two weeks prior to the sport's start date to allow for processing time. Forms submitted late may risk the student beginning their sport on day one.
