Registration Documents Required

Registration of a student is completed through Online Registration.

Step-by-step instructions for registering a student are provided. For assistance or questions,
please email registration@esmonline.org.

The following are the documents required at the time of registration.  You will be asked to upload these documents during the online registration process.

PROOF OF CHILD’S AGE

PROOF OF IMMUNIZATIONS
  • Certified Birth Certificate or
  • Baptismal Certificate or
  • Passport
  • Proof of required immunizations signed by a physician or
  • Copy of immunization record from prior school

 

PROOF OF RESIDENCY

You must provide at least one item from the following:

Owners and Renters must provide the following:

OWNERS

  • Deed or
  • Tax Bill or
  • Mortgage Statement or
  • Real Estate Closing Statement
  • Photo Identification (Driver’s license, government issued identification, passport)

RENTERS

  • Lease or Rental Agreement and
  • Registration Affidavit Form.

The Registration Affidavit Form IS NOT required if you reside in Pine Hills or Heritage Homes.

Two current Utility Bills (PSEG, National Grid, Cable, Water, Oil/Fuel) indicating the address of residence and within the last thirty (30) days, and/or

Documents issued by Federal, State or local agencies (Voter Registration, Federal Office of Refugee Resettlement, local social service agency).

 

PARENTAL CUSTODY – ACCEPTABLE DOCUMENTS

Court Ordered Custody/Guardianship – If a court has issued an order pertaining to the custody or guardianship of a child, the court papers must be presented at the time of registration.

Statements of Custody – If a child is living with persons within the District other than the child’s biological or adoptive parents, and no court order has been issued awarding custody or guardianship to the person(s) with whom the child is living, the District requires the parent(s) and person(s) in parental relation to provide statements verifying the transfer of parental custody and control.

No Contact, Order of Protection – If a court has issued an order stipulating that there is to be no contact to a child, the court papers must be presented at the time of registration.

OWNERS

  • de hipoteca o declaración de

  • Identificación con foto (licencia de conducir, identificación emitida por el gobierno, pasaporte)

RENTERS

  • Contrato de Arrendamiento o Alquiler y
  • Formulario de Declaración Jurada de Registro.

El Formulario de Declaración Jurada de Registro NO ES necesario si reside en Pine Hills o Heritage Homes.

Dos facturas de servicios públicos vigentes (PSEG, red nacional, cable, agua, petróleo/combustible) que indiquen la dirección de residencia y dentro de los últimos treinta (30) días, y/o

Documentos emitidos por agencias federales, estatales o locales (Registro de Votantes, Oficina Federal de Reasentamiento de Refugiados, agencia local de servicios sociales).

 

CUSTODIA DE LOS PADRES – DOCUMENTOS ACEPTABLES

Custodia/tutela ordenada por el tribunal – si un tribunal ha emitido una orden relacionada con la custodia o tutela de un niño, los documentos judiciales deben presentarse en el momento del registro.

Declaraciones de custodia – Si un niño vive con personas dentro del Distrito que no sean los padres biológicos o adoptivos del niño, y no se ha emitido una orden judicial que otorgue la custodia o tutela a la(s) persona(s) con la(s) que vive(n), el Distrito requiere que el(los) padre(s) y la(s) persona(s) en relación parental(es) proporcionen(n) declaraciones que verifiquen la transferencia de la custodia y el control parental.

Orden de protección sin contacto – si un tribunal ha emitido una orden que estipula que no debe haber contacto con un niño, los documentos judiciales deben presentarse en el momento del registro.
